Dr. Grushka Answers Your Questions about Her Office
Dr. Grushka has compiled some commonly asked questions about our Toronto office. If you have more questions, please call us during office hours.
Where can you park?
There is parking available between 9 AM and 4 PM on the street in front of our location. Parking is metered. Additional parking can be found in a few Green P parking lots located just east of our location across from the Starbucks, directly across from our location beside the Pizzaiolo and just west of our location on the south side.
How much are appointments?
Appointments with Dr. Grushka are not covered under OHIP. However, if you have dental insurance, we will be happy to provide you with dental insurance forms for your completion and submission. The fee for an Initial appointment is $575.00 (Fees subject to change). Follow up appointments are $375.00 each (Fees subject to change). Dental insurance forms will be provided to you after each visit for submission.
Is it covered under OHIP?
Unfortunately, appointments are not covered under OHIP. However, most tests/additional referrals are done through the medical system and are covered under OHIP.
How do I prepare for my appointment?
If you are using medication, please come prepared with a list for patient form fill-out. Also, if you have had previous tests performed in relation to the issue you are being seen for, medical reports would be much appreciated either at the time of your appointment or prior from your physician’s office. If you have a TMJ issue, please bring any mouth guards/appliances you have with you to your appointment. If possible, please also have the contact information for your health care practitioners ready.
Where is the office located?
Our office is located at 974 Eglinton Avenue West in Toronto. Please ensure your GPS system does not take you to Mississauga, where there is also a 974 Eglinton Avenue! We are on the north side just west of Bathurst Street.
What forms of payment are taken?
We accept payment by credit card (MasterCard, Visa, AMEX), debit card, cash and cheques.
